A few things that help us run on time — and serve you well.

Appointments
  • Please arrive 5–10 minutes early.
  • Patients more than 10 minutes late may need to reschedule.
  • Cancellations require 72 hours notice — $50 no-show fee otherwise.
Payments
  • Copays and deductibles are due at check-in.
  • We accept cash, check, and credit/debit cards.
  • Self-pay rates available — please ask the front desk.
Medication refills
  • Request refills through your pharmacy.
  • Typical response time is 72 hours.
  • Annual labs may be required for certain medications.
Routine concerns
  • Call during office hours.
  • Expect a response within 48–72 hours.
  • Use the portal for non-urgent messages.
Urgent issues
  • If you have a medical emergency, call 911.
  • For urgent issues, visit the nearest ER or urgent care.
  • Notify our office afterwards so we can coordinate care.
Medical records
  • Free fax service to your primary care physician.
  • Printed copies are $0.75 per page.
  • A Records Request Form is required.
