We make becoming a new patient straightforward. Once we have your insurance details and prior records, our team will call to confirm details and book your appointment.
We accept most major insurance plans, including Medicare and Medi-Cal. Please confirm in-network status with your carrier or our office.
Submit a records release form via fax or mail and we'll request your prior notes, labs, and imaging from your previous providers.
Once we receive and review your medical records, we'll call you to confirm details and book your first appointment.
Come prepared, leave with a plan.
For your first appointment, please bring the following so we can make the most of our time together.
- Photo IDDriver's license, state ID, or passport.
- Insurance card(s)Primary and secondary, if applicable.
- Medication listIncluding dose, frequency, and duration.
- Payment methodCash, check, or credit/debit card.
- Prior labs & imagingIf we haven't already received them.
- List of questionsBring concerns or symptoms to discuss.
We're in-network with most major plans.
If your plan isn't listed, please call our office — we may still be able to see you, and self-pay rates are available.
